Western hostility toward Tehran stems from its challenge to the global order rather than its nuclear program, Iran’s supreme leader said, arguing that the core dispute is ideological.
The problem between the Islamic Republic and Western powers “is not the nuclear issue,” but opposition to Iran’s plan “to establish a national and international Islamic order,” Ali Khamenei said in a message issued Saturday to the annual meeting of Islamic student associations in Europe.
He framed the confrontation as resistance to an “unjust global order and the system of domination.”
“The heavy assault of the US army and its disgraceful appendage in the region was defeated by the initiative, courage and sacrifice of Iran’s young people,” Khamenei said in a reference to recent regional conflicts.
Costs of confrontation
The remarks came after a 12-day war that left Iran with significant losses among senior military commanders and caused damage to nuclear, military and security infrastructure. Critics say the confrontation with the West, shaped by Khamenei’s policies over more than three decades, has imposed heavy costs on the population, with sanctions crippling large parts of the economy, weakening the currency and driving up the cost of living.
Despite this, senior officials have continued to defend the strategy. On Friday, Foreign Minister Abbas Araghchi acknowledged that sanctions have contributed to economic disorder but portrayed them as a factor behind Iran’s industrial “independence” and the development of its defence and missile sectors.
Speaking earlier in Isfahan, Araghchi said Iranians must accept the reality of sanctions and “accept that it is possible to live with sanctions.” He added that while sanctions have costs, he also knew their “blessings,” comments that triggered a wave of public criticism from citizens who said officials were disconnected from daily economic hardship.
Nuclear dispute still central
Khamenei’s insistence that the nuclear issue is secondary contrasts with Tehran’s recent remarks that reports by the International Atomic Energy Agency helped pave the way for Israeli and US attacks on Iran’s nuclear facilities.
On June 12, the IAEA’s board of governors adopted a resolution accusing Iran of breaching safeguards by accumulating highly enriched uranium and restricting inspectors’ access, and called on Tehran to resume full cooperation.
The supreme leader’s comments also come as Iran-backed groups across the region are widely viewed as major sources of instability in the Middle East, complicating Tehran’s efforts to cast its confrontation with the West as purely ideological.
Iran would respect Iraq’s sovereignty but stood ready to act if the Iraqi government sought assistance, Mohammad Kazem Al-e Sadegh said in an interview reported by the ISNA news agency.
“Iran is ready to protect the political system in Iraq from collapse if officially requested,” he said, without elaborating on what such protection would entail.
Iran supports Iraqi groups through financing, training, and arms, primarily focusing on Shia militias that are often integrated into the official Popular Mobilization Forces (PMF).
This support helps groups like the Badr Organization and Kata'ib Hezbollah exert military and political influence, though some factions like Harakat Hezbollah Al-Nujaba have focused more on military operations. The support allows Iran to pursue its regional objectives, gain influence, and destabilize Iraqi politics while coordinating attacks against US forces.
Armed groups and ‘independent decisions’
Al-e Sadegh addressed recent Iraqi initiatives aimed at consolidating weapons under state control, a sensitive issue given the power of Iran-aligned armed factions. The Iran-backed groups, he said, were hesitant about weapons monopolisation due to concerns over its consequences, but insisted they had reached a level of maturity that allowed them to make decisions independently.
“Our relationship with these groups is longstanding, but they have reached a stage where they can decide for themselves,” he said, rejecting descriptions of the factions as Iranian proxies.
On regional tensions, the ambassador said Iran was “fully prepared” to respond to any hostile action by Israel, adding that Israel had sought US mediation to secure a ceasefire.
Iraq has balanced relations with both the United States and Iran, but faces mounting risks to its financial system if it falls foul of global sanctions regimes.
Hezbollah and the Houthis are key members of a broader network of Iran-backed groups across the region.
Iran views Iraq as a strategic economic and political partner amid Western sanctions, while Baghdad remains wary of being drawn into US efforts to squeeze Tehran and its regional allies.
The investigation documented at least six extrajudicial killings and 11 cases of detention involving Afghans expelled from Iran, though the outlet said the true number of victims is likely higher. Families of those killed or detained have blamed the Taliban, describing a recurring pattern of violence against people forcibly returned from Iran.
The findings come amid a sharp rise in deportations following the announcement of a ceasefire in a 12-day war in June by US President Donald Trump. According to the International Organization for Migration, more than 500,000 Afghan nationals were expelled from Iran between June 24 and July 9 alone. Iran’s Interior Minister Eskandar Momeni later said about 1.5 million Afghans had been deported this year.
Targeted after forced return
Many deportees, Afghanistan International said, were refugees at immediate risk, including former government officials, journalists and civil society activists who fled Afghanistan after the Taliban takeover in 2021. Several were killed or detained shortly after their return.
Among the cases documented was the killing of Kamin Jan, a former interior ministry employee, who was shot dead in Takhar province in October 2025, weeks after being deported from Iran. Two weeks earlier, the body of another former Afghan soldier who had also been expelled from Iran was found in Farah province.
In another case, the body of Gul Ahmad, a former interior ministry employee, was discovered in Farah in October 2025 after he was taken from his home by armed men carrying Taliban intelligence identification cards. His family said he was held for more than three months before his body was returned.
A similar incident occurred in Kabul, where the body of Abdulwali Naimi, a former Afghan special forces officer from Panjshir province, was found in September 2025, two weeks after his deportation. International organizations, including the United Nations, have reported sustained retaliatory attacks against former Afghan security personnel since the Taliban takeover.
One day after deportation
Some deaths followed almost immediately after expulsion. In July 2025, Ezzatullah, a former commander affiliated with the Junbish-e Milli (National Islamic Movement of Afghanistan) party in Sar-e Pol province, was killed just one day after returning from Iran. Ezzatullah had fled to Iran after the Taliban takeover but was forced to leave when his visa expired, a party spokesperson said.
Civil society activists were also targeted. Hamza Ulfat, a former Taliban detainee and civil activist, was shot while fleeing Taliban forces shortly after crossing back from Iran and later died from his wounds.
Detention, torture and silence
Beyond killings, the investigation documented multiple cases of detention and torture. Khal Mohammad, a former Afghan army colonel deported from Iran in October 2022, said Taliban forces tortured him and his elderly mother in separate incidents. Afghanistan International said it reviewed videos and medical records showing signs of abuse.
Despite repeated warnings from human rights groups, Iran has continued deportations. Filippo Grandi, the UN high commissioner for refugees, has called forced returns from Iran and Pakistan alarming, warning they return people to unsafe conditions.
killings extend inside Iran
The investigation comes as Afghan opposition figures have also been targeted inside Iran. On Wednesday, former Afghan police general Ikramuddin Sari was shot dead by masked assailants near his home in Tehran, the second such killing in under four months. Sari, who had served as police commander in Baghlan and Takhar provinces, fled to Iran after 2021 and was an outspoken critic of the Taliban.
His killing followed the September shooting of Maroof Ghulami in Mashhad. People close to both men blamed the Taliban, with an Afghan military source describing the attacks as the start of Taliban “extraterritorial assassinations.” Iranian authorities have not publicly commented on Sari’s killing or released findings from the Ghulami investigation.
Rights advocates say the pattern of violence raises serious concerns about violations of the international principle of non-refoulement, which bars returning individuals to places where their lives or freedom are at risk.
Doctors working in public and private psychiatric hospitals told Tehran-based daily Etemad that they are seeing growing numbers of patients with no prior mental illness who develop severe psychotic symptoms after using narcotics and synthetic stimulants.
Many require emergency hospitalization, and some go on to develop lasting psychiatric disorders.
A senior psychiatrist at a 100-bed psychiatric hospital in southwest Tehran said that typically 15 to 20 percent of inpatient beds are occupied by patients admitted after drug-induced psychosis.
“Acute symptoms mean a crisis – self-harm, violence toward others, suicidal behavior, severe delusions and hallucinations,” the doctor said, adding that most arrive in an aggressive and disoriented state.
“These patients must often be physically restrained during transfer because they pose an immediate danger to themselves and others,” the psychiatrist said, noting that many families cannot afford private ambulances and that treatment costs are not covered by insurance.
Teens increasingly at risk
The most troubling shift is the age profile, doctors say. While hospitals previously treated mostly adult men, adolescents now make up a growing share of emergency admissions.
“More than half of pediatric psychiatric admissions are teenagers who have used heavy substances,” one psychiatrist said, adding that cannabis derivatives are now “circulating like chewing gum” among youths.
Another doctor described a rise in cases involving homemade drug mixtures and hallucinogens such as LSD and mushrooms. “Some teenagers believe they are entering a spiritual world,” he said. “Instead, they arrive with persistent hallucinations that can last for years.”
A resident psychiatrist at a public hospital said that in the past year, 10–20% of emergency psychiatric visits involved first-time psychosis triggered by drug use. “Some test negative for substances by the time they reach us, yet they are hallucinating and delusional,” he said. “Even if symptoms subside after treatment, continued drug use puts them at high risk of developing chronic disorders.”
Warnings of long-term fallout
Stimulants such as methamphetamine and cocaine sharply increase the risk of severe behavioral changes, Mohammadreza Shalbafan, head of mental health and addiction at the Iranian Ministry of Health told Etemad. “These reactions are not limited to people with a psychiatric history,” he said. “No drug user is immune.”
Toxicologist Kambiz Soltaninejad, speaking at a medical workshop in September 2025, warned that high-potency cannabis strains pose the greatest risk. “With teenage use rising, we should expect a schizophrenia tsunami in the near future,” he said.
Advocacy groups say the warnings confirm what they have long observed. Rising use of hallucinogens has already pushed hospital admissions higher, Tayebeh Dehbashizadeh, head of the Association for the Support of Schizophrenia Patients said.
“Global estimates suggest one percent of any population has schizophrenia,” she said. “In Iran, that number is climbing.”
Doctors caution that hospital data capture only the most severe cases. “We are seeing just the tip of the iceberg,” one psychiatrist said. “Many more are deteriorating quietly at home, unnoticed – until it’s too late.”
Cases that haunt clinicians
Doctors interviewed by Etemad described cases that continue to trouble them years later. One psychiatrist recalled a 27-year-old man who developed vivid, shifting hallucinations after consuming a small amount of psychedelic mushrooms. He remained trapped in a fantasy world for nearly two years before briefly regaining clarity, only to spiral into severe post-traumatic stress and later schizophrenia after renewed stress, requiring lifelong treatment.
Another case involved a 16-year-old girl from a family affected by addiction, who began injecting drugs in early adolescence. She was admitted after fearing a fatal overdose and asking for help but disappeared days after being discharged from a pediatric ward.
“No one knows what happened to her after she walked out of the hospital,” a doctor said.
Clinicians also described patients who returned repeatedly after discharge, only to die from overdoses weeks later. “Some survive the psychosis, but not the next relapse,” one psychiatrist said. “That is the most devastating part.”
Arash Darbandi, a photographer from Ahvaz in southwestern Iran, told the Ukrainian outlet that he arrived in St. Petersburg on a tourist visa and supported himself by taking photographs of passersby.
“I took photos of anyone who was dressed in colorful clothes. If they liked the photo, they paid me 1,000 rubles ($10-11),” he said.
Although trained as a petroleum engineer, Darbandi said photography had been his main livelihood.
He acknowledged knowing that Russia was at war with Ukraine but said the conflict initially felt distant. That changed after an encounter with police that led to his detention.
According to Darbandi, he was arrested following an altercation with a police officer and taken to a military facility on Ligovsky Prospekt.
He said authorities gave him a stark ultimatum: prison or the battlefield.
“They told me that I either had to go to prison for three to five years, or go to the war for one year,” he said.
When he objected and argued that deportation should be the maximum punishment for a foreigner, he said officers replied: “This is Russia, and you must go to war.”
Iran emerged as one of Russia’s key military backers since Moscow launched its full-scale invasion of Ukraine in February 2022. Tehran has been accused of supplying Russia with hundreds of Shahed-series attack drones, which have been widely used against Ukrainian cities and infrastructure.
Western governments and Kyiv say Iranian-made drones have played a central role in Russia’s aerial campaign, a charge Iran has repeatedly denied or downplayed.
Self-harm to avoid war
Darbandi said he was held in barracks for months before being sent to a training center near Belgorod.
Fearing deployment, he said he deliberately injured himself, breaking his arm, but was still not exempted from service despite Russian law.
“I had never even held a knife,” he said, stressing he had no military background.
He described training as minimal and coercive, saying recruits were treated as expendable.
“They didn’t treat us as humans; they only saw us as expendable and just wanted to send us to the front so that the Russians could live safely,” he said.
Darbandi said foreign nationals—including Iranians, Africans, Arabs, Kenyans, and Colombians—were segregated from Russian soldiers.
“Foreigners have no rights at all; at any moment, they can take whatever they have,” he said.
He said he was later wounded during a Ukrainian drone strike and captured after being left without assistance for days.
Reflecting on his situation, Darbandi said he feels guilt over his forced participation and urged others not to cooperate with governments he accused of exploitation.
“Never help countries like Russia, Iran, and countries that support terrorists. Please stop the war.”
The account has not been independently verified. Iranian, Russian, and Ukrainian officials have not publicly commented on the case.
Recruitment drive in Tehran
Earlier this month, flyers circulated near the Russian Embassy in Tehran that invited men to enlist in the Russian army for promises of dollar payments and contracts “directly under the Ministry of Defense of the Russian Federation.”
The Russian embassy denied any connection to the leaflets.
The flyers targeted men aged 18 to 45 and offered starting bonuses of $15,000 to $18,000 and monthly salaries of $2,500 to $2,800, along with free housing, medical care, and military uniforms.
Tehran-based outlet Rouydad24 said the leaflets directed readers to a Telegram channel that had published multilingual posts in Persian, Russian, Arabic, and English, describing the campaign as a “state-supported initiative.”
The Iranian report compared the flyers to similar alleged recruitment efforts in India, Nepal, Sri Lanka and several African countries, which foreign media have described as part of Moscow’s drive to attract foreign fighters amid heavy losses in Ukraine.
